Patient Monika, aged 41, presented for cervical spine surgery (cervical discectomy) to remove a fragment of disc protrusion at segment C5-C6 via a lateral approach.

02.09.2020 Magnetic resonance imaging of segment C1-Th4

1. at the level of C3-C4 and C5-C6, small central protrusions of the intervertebral discs modelling the meningeal sac, without signs of collision of spinal nerves
2. at the level of C5-C6, wide, central herniation of the nucleus pulposus of approximately 3.5 mm, compressing the meningeal sac with the possibility of collision of spinal nerves, especially on the right side, causing stenosis of the spinal canal to the sagittal dimension of 10 mm
3. At the C6-C7 level, a wide disc protrusion compressing the meningeal sac with possible collision of spinal nerves on the right side.
4. At the level of C7-Th1, there is a wide disc protrusion that models the meningeal sac, with a possible collision of the position of the spinal nerve on the right side.
5. There is a flattening of the physiological lordosis of the cervical spine, initial degenerative changes and degenerative changes of the intervertebral discs C5-C6 and C6-7 in the form of their partial dehydration.

1. physiological cervical lordosis abolished in the supine position
2. slight arthrosis of the anterior medial psoas joint
3. circular disc protrusion in segment C5-C6 causing slight central stenosis of the spinal canal (medial a-p dimension of the meningeal sac 11 mm)
4. also trace central C4-C5 protrusion and small C6-C7 circular protrusion.
5. No other changes are found in the examined spinal segment. There is no arthropathy
6. No orifice stenosis found. Homogeneous structure of the cervical spinal segment. Apart from a small focus of degenerative bone marrow edema in the posterior basal margin of the C6 vertebral body, no bony swelling changes are seen.
